Position Summary:
The Behavioral Health Therapist with psychosocial support skills will provide comprehensive emotional, social, and practical support to individuals living with HIV. This role involves facilitating peer support groups, conducting individual counseling sessions, and ensuring clients are connected to critical resources, including housing, food assistance, and workforce development programs. The therapist will also participate in interdisciplinary team meetings, manage Ryan White eligibility documentation, and collaborate with clients to develop and update personalized treatment plans.
Key Responsibilities include:
Facilitate Peer Support Groups:
- Organize and facilitate bi-monthly group sessions to provide a safe, supportive environment for individuals living with HIV to share experiences, learn coping strategies, and build a sense of community.
- Foster a non-judgmental, empathetic atmosphere to promote open communication and emotional support.
Individual Counseling:
- Provide individual counseling to clients to address the emotional and social challenges that accompany living with HIV.
- Use evidence-based therapeutic techniques to support clients in managing stress, grief, mental health issues, and other psychosocial concerns.
- Key knowldege with clients dealing with various substance use disorders is essential.
Case Management/Linkage to Resources
- Assist clients in accessing essential community resources, including housing, food assistance, mental health services, and workforce development programs.
- Work closely with external agencies and service providers to ensure clients receive the support they need.
Documentation and Reporting:
- Collect and update eligibility documentation to ensure clients continue to qualify for necessary services.
- Maintain accurate and up-to-date records of client eligibility status and related information in eCW and CAREWare.
- Prepare regular reports on client progress, service outcomes, and program effectiveness.
Treatment Planning and Updates:
- Work with clients to develop and execute individualized treatment plans that address their psychosocial needs, goals, and progress.
- Regularly review and revise treatment plans to ensure they are aligned with clients' changing circumstances and needs.
Interdisciplinary Collaboration:
- Actively participate in interdisciplinary team meetings, providing insights into the psychosocial needs of clients.
- Collaborate with healthcare providers, case managers, and other team members to ensure comprehensive care for individuals living with HIV.
Qualifications:
- Master’s degree in social work, LICSW required or NCAC II – DC, MD Licensed preferred.
- Minimum of 2 years of experience in social work, substance use disorders, social services, or in a related therapy or clinical role.
- Proficiency in working with electronic health records (experience with multiple systems a plus).
- Strong attention to detail and organizational skills.
- Excellent interpersonal and communication skills.
- Ability to manage confidential information with discretion and professionalism.
- Experience working with diverse populations in community-based or health care settings preferred.
- Bilingual in English and Spanish is a plus.
Working Conditions:
- Occasional local travel for outreach or community engagement events may be required.
- Must be able to sit, stand, and operate a computer for extended periods.
