An exciting opportunity for a Licensed Vocational Nurse to join a specialized team within a large healthcare system in San Antonio, Texas. The candidate will work collaboratively with our cross-functional team to coordinate care for patients with various chronic illnesses.
We are interested in employing a detail oriented, well-organized LVN with experience in care coordination for complex patients. Candidate must possess the ability to work well on a team and be comfortable engaging with medical providers, pharmacists, other medical staff, and patients.
Schedule is full-time weekdays. Candidate must be able to reliably commute to our San Antonio medical center area office.
Job duties include but are not limited to:
- Reconcile medications and supplies.
- Assist in the coordination of patient care and nursing activities including proper documentation of medical history and vitalization.
- Maintain accurate and timely data entry in electronic medical records.
- Respond to phone calls from patients, medical providers, and other medical staff members.
- Learn the subject matter and be part of a team delivering quality specialty care.
