Job Description:
We are seeking skilled and dedicated Licensed Vocational Nurses (LVNs) to join our growing team. As an LVN with Prime Home Health, you will provide direct nursing care to patients in their homes under the supervision of an RN Case Manager. You will play a key role in supporting patient recovery, promoting independence, and ensuring continuity of care.
Responsibilities:
- Provide skilled nursing care to patients in their home setting
- Follow individualized care plans and physician orders
- Monitor, document, and report patient progress and changes in condition
- Administer medications and treatments as prescribed
- Collaborate with the interdisciplinary care team to ensure the highest quality of care
- Educate patients and families on care plans, medications, and disease management
Qualifications:
- Current and Valid LVN License in the state of California
- Current CPR/BLS certification
- Current TB or Chest X-ray if applicable
- Reliable transportation and Valid Driver’s License
- Excellent communication and documentation skills
- Compassionate and professional demeanor

At Prime Home Health, we believe in supporting new graduates as they begin their nursing careers. Our agency provides a strong foundation and valuable hands-on experience to help you grow professionally and confidently transition into the field.
We offer paid training and opportunities under the Flex Waiver Program to gain additional clinical experience across a variety of patient cases. After one year of employment, we can also assist you in obtaining tracheostomy and ventilator certification, if desired.

COVID-19 Considerations:
At Prime Home Health, employee and patient safety are our top priority. We follow strict safety protocols and provide PPE such as gloves, masks, and face shields upon request. Hired nurses will not work with COVID-19 positive patients.
To further reduce exposure, our LVNs typically work exclusively with one patient, limiting interactions across multiple homes or staff.
