What You'll Be DoingLead Patient Care
- Perform comprehensive assessments and develop individualized care plans
- Manage ongoing visits and patient education
- Support patients and families throughout the recovery process
- Coordinate care across the healthcare continuum
- Deliver exceptional care that promotes independence and quality of life
Use Your Clinical Expertise
- Manage chronic conditions and complex medical needs
- Perform OASIS assessments and clinical documentation
- Collaborate with physicians and interdisciplinary teams
- Identify changes in condition and intervene proactively
- Help reduce avoidable hospitalizations and emergency room visits
Build Relationships That Matter
- Become a trusted resource for patients and families
- Provide encouragement, education, and support

- Help patients remain safe and independent in their homes
QualificationsRequired
- Active Minnesota RN license
- Minimum of three years of RN experience
- Strong assessment and communication skills
- Reliable transportation and valid driver's license
- Passion for independent, relationship-based nursing
Preferred
- Home health experience
- OASIS experience
- Previous case management experience
