Triage Nurse – Key Responsibilities
Under the general supervision of a physician, the Triage Nurse provides high-quality, patient-centered nursing care and supports efficient clinical operations. The nurse serves as a key liaison between patients, providers, and the healthcare team to ensure timely, coordinated care.
- Provide professional nursing care to assigned patients, demonstrating accountability for clinical judgment and maintaining quality standards within the scope of practice and organizational policies.
- Perform triage assessments of patient concerns via telephone or in person, determining appropriate levels of care and providing nursing advice or clinical direction as needed.
- Process medication refill requests in accordance with established clinical protocols and provider guidance.
- Facilitate patient access to care by coordinating urgent or follow-up appointments, assisting with provider referrals, and communicating effectively with external clinics.
- Obtain and manage prior authorizations for medications, diagnostic tests, and procedures to support timely treatment and continuity of care.
- Accept and implement provider orders (written or verbal) from authorized clinicians, ensuring accuracy and adherence to policy.
- Accurately document and report patient symptoms, clinical findings, and communications in the electronic health record (EHR).
- Maintain comprehensive and up-to-date patient records, ensuring all documentation is clear, concise, and compliant with regulatory standards.
- Collaborate effectively with physicians, nursing staff, and other members of the healthcare team to promote coordinated and efficient care delivery.
- Support daily clinic operations by performing additional duties as assigned to ensure smooth workflow and a positive patient experience.
QUALIFICATIONS
· Current unrestricted Minnesota Registered Nurse Licensure.
· Excellent written and verbal communication skills required.
· Experience with electronic medical record systems preferred.
· Minimum one-year RN experience preferred.
