We are seeking a dedicated, compassionate Certified Nursing Assistant (CNA) to join our hospice team serving patients in a skilled nursing facility in Farmerville, Louisiana.

Position Summary:
As a Hospice CNA, you will work alongside our interdisciplinary hospice team to provide compassionate, patient-centered care to individuals receiving hospice services within a nursing home setting. Your role is essential in promoting comfort, preserving dignity, and supporting both patients and their loved ones through every stage of the hospice journey.
The ideal candidate is caring, dependable, observant, and committed to treating every patient as if they were a member of their own family.
Responsibilities
- Provide exceptional personal care and assistance with Activities of Daily Living (ADLs), including:
- Bathing
- Dressing
- Grooming
- Toileting
- Oral hygiene
- Mobility and transfers
- Assisting patients with repositioning, ambulation, wheelchairs, and other mobility equipment while promoting comfort and safety.
- Observe, monitor, and promptly report changes in patient condition to the hospice nurse.
- Accurately document patient care and observations using Electronic Medical Records (EMR).
- Measure and record vital signs as directed.
- Assist with catheter care, skin care, incontinence care, and other personal care needs within the CNA scope of practice.
- Support patients with Alzheimer's disease, dementia, memory loss, and other cognitive impairments using compassionate, individualized approaches.
- Help maintain a clean, safe, and comfortable patient environment.
- Provide emotional support, companionship, and reassurance to patients and their families.
- Work collaboratively with Registered Nurses, Social Workers, Chaplains, Hospice Aides, Facility Staff, and Physicians to ensure each patient's plan of care is followed.
- Respond appropriately to emergencies and follow all safety procedures, infection control practices, HIPAA regulations, and hospice policies.
- Demonstrate professionalism, empathy, respect, and compassion in every patient interaction.
Qualifications
- Current Louisiana Certified Nursing Assistant (CNA) certification.
- Current CPR certification (or ability to obtain upon hire).
- Previous experience in hospice, long-term care, skilled nursing, assisted living, or home health is preferred.
- Experience caring for geriatric patients and those with Alzheimer's disease, dementia, or other chronic illnesses is highly desirable.
- Knowledge of hospice philosophy and end-of-life care is preferred.
- Strong communication and interpersonal skills.
- Ability to accurately complete electronic documentation in an EMR system.
- Ability to safely assist patients with transfers and mobility, including use of Hoyer lifts and other assistive devices.
- Ability to lift, bend, stand, and perform the physical requirements of patient care.
