Elevation Staffing Solutions is building a network of skilled, independent Home Health Physical Therapists to fill visits for home health agencies across the Inland Empire. As an independent contractor with ESS, you'll deliver personalized physical therapy services directly in patients' homes, promoting recovery, mobility, and independence across diverse populations — geriatric, pediatric, neurological, orthopedic, and post-surgical. What You'll Do
- Assess patients' physical conditions and develop customized treatment plans tailored to individual needs and goals
- Provide hands-on therapy interventions such as therapeutic exercises, manual therapy, trigger point therapy, and mobility training
- Coordinate discharge planning and collaborate with the agency's care team to ensure seamless patient transitions across care settings
- Document patient progress accurately per each agency's EHR system and clinical documentation standards
- Educate patients and families on home exercise programs, safety precautions, and ongoing care strategies
- Monitor patient response to treatment and adjust care plans accordingly
What We're Looking For
- Strong knowledge of physiology, anatomy, and medical terminology relevant to physical therapy practice
- Experience with geriatric, pediatric, neurological, orthopedic, or outpatient rehabilitation populations
- Comfort with discharge planning, treatment plan development, and individualized rehab strategies
- Familiarity with EHR/EMR documentation systems
- Strong communication skills for coordinating with agency care teams, patients, and families
- Home health or community care experience strongly preferred
- Operate your own business entity (LLC, S-corp, or sole proprietorship) with active professional liability insurance — ESS partners with independent clinicians, not employees
- OASIS and Start of Care (SOC) assessment experience welcomed, but not required — we're happy to support clinicians building this skill set
