As a member of the team it is your opportunity to play a vital role in providing wellness coaching, care coordination and support navigating services to children, youth and families. You will help connect individuals to essential resources and care, making a meaningful impact everyday. You will deliver ongoing community based assessment, treatment and supports for children and youth experiencing or at-risk for severe emotional disturbance assisting as they and their families traverse healthcare, community systems and education systems.
Summary of Duties

Identify youth and family needs, strengths and goals while simultaneously addressing barriers to accessing services

Make referrals and follow up on services

Support communication between providers and members of the youth’s team

Support youth with goal-setting, organization, problem-solving and self-advocacy

Provide support in schools, homes, and community settings

Document services, referrals, coordination, and progress

Collaborate with clinicians and other CYFS staff

The starting pay rate for this position is $24.87 per hour.
Summary of Qualifications
Bachelor's degree in related field
Experience with providing support to parents around a myriad of childhood issues including but not limited to, development, behavioral management, and attachment
Experience interfacing with community partners
Ability to work cooperatively and responsibly as a member of a team from diverse backgrounds
Abide by confidentiality policies
Due to the rural nature of our catchment area, a valid driver’s license, excellent driving record and access to a safe, reliable, insured vehicle is required
Work takes place in a variety of office and community settings. Frequent travel between sites over the course of a week. Extended use of a computer.
