Job Overview

Duties
- Conduct thorough patient assessments, including physical examinations and vital signs monitoring, to determine health status and develop individualized care plans.
- Manage acute and chronic conditions utilizing evidence-based practices, including medication administration, IV infusion therapy, and wound suturing.
- Perform diagnostic evaluations such as laboratory specimen collection, sonography, spinal taps, and medical imaging interpretation to inform treatment decisions.
- Provide specialized procedures including botulinum toxin treatments, dermal fillers, esthetic laser treatments, catheterizations, and airway management techniques.
- Collaborate with healthcare teams on discharge planning, case management, utilization review, and telehealth consultations to optimize patient outcomes.
- Maintain accurate medical documentation within electronic health record (EHR) systems like Epic or eClinicalWorks while adhering to HIPAA regulations.
- Educate patients and their families on health coaching topics such as nutrition, medication adherence, disease prevention, and post-acute care strategies.
Requirements
- Valid Nurse Practitioner license with current state certification; experience in primary care or specialized fields such as geriatrics or pediatrics is preferred.
- Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth for efficient documentation and patient tracking.
- Critical care experience in settings such as ICU or Level I Trauma Center enhances your ability to manage complex cases involving trauma or infectious disease care.
- Hands-on experience with procedures like IV insertion, phlebotomy, sterile processing techniques, and suturing is highly desirable.
- Knowledge of CPT coding (Current Procedural Terminology), ICD-9/10 coding systems for accurate billing and documentation purposes.
- Strong understanding of HIPAA compliance standards along with excellent patient assessment skills across diverse populations including assisted living residents and individuals with developmental disabilities.
