Duties
- Conduct thorough patient assessments including vital signs, physiology evaluation, and medical records review to determine care needs.
- Coordinate multidisciplinary care plans for diverse populations such as pediatrics, geriatrics, behavioral health, and trauma patients in outpatient settings.
- Manage utilization review and medical-surgical case management to ensure appropriate level of care and resource allocation.
- Facilitate discharge planning by collaborating with physicians, social workers, and community providers to arrange post-acute care services.
- Oversee diagnostic evaluations and medical records documentation to support clinical decision-making and compliance with HIPAA regulations.
- Perform intake assessments for outpatient services, ensuring smooth patient onboarding and service delivery.
- Utilize clinical expertise to guide patient care strategies effectively.
Qualifications
- Valid nursing license with current registration in the relevant state or jurisdiction.
- Strong background in hospital medicine, medical-surgical units, post-acute care settings, outpatient clinics or managed care environments.
- Demonstrated knowledge of physiology, medical terminology, patient assessment techniques, and discharge planning processes.
- Experience with utilization management, diagnostic evaluation coordination, and case management procedures.
- Familiarity with HIPAA compliance standards and medical records documentation practices.
- Excellent communication skills with the ability to collaborate effectively across multidisciplinary teams.
