Job Summary
We are seeking a compassionate and dedicated Home Care Nurse to provide high-quality medical care to patients in their homes. The ideal candidate will possess a strong background in home care and be skilled in various nursing practices, ensuring the comfort and well-being of our clients. This role requires a proactive approach to patient care, excellent communication skills, and the ability to work independently.
Responsibilities
- Create an initial plan of care for expected activities of the PCA for all new patients
- Conduct follow up visits to ensure the PCA is following the plan of care
Qualifications
- Valid nursing license in the state of practice.
- Experience in home care or similar settings is preferred.
- Knowledge of occupational health practices, hospital medicine, and senior care principles.
- Familiarity with ICD-9 coding for accurate documentation and billing processes.
- Skills in family planning, critical care transport, and triage are advantageous.
- Strong interpersonal skills with the ability to build rapport with patients and families.
- Excellent problem-solving abilities and attention to detail.
- Ability to work independently while managing multiple priorities effectively.
