POSITION SUMMARY:
Under the supervision of the Associate Director of Quality, the QAI Coordinator facilitates clinical improvement projects, assists with improvement measurement and analysis, and assists with reporting of clinical outcomes and improvements. Integrates improvement support activity with related regulatory and accreditation standards to promote compliance.
DUTIES AND RESPONSIBILITIES:
- Utilize quality improvement techniques to improve customer care and clinical outcomes.
- Manage clinic initiatives to improve UDS measurement and analysis, as well as reporting of clinical outcomes and improvements.
- Incorporate regulatory accreditation requirements into improvement projects.
- Function as the lead to abstract data and generate reports from medical records for key clinical improvement projects.
- Identify trends and conduct relevant analysis from AHSFHC’s clinical performance data.
- Assist with identifying benchmarks and comparisons for clinical outcomes measures.
- Act as a lead resource to identify trends in healthcare quality improvement, incorporates into planning and development of improvement initiatives, and maintains current knowledge of regulation and standards.
- Advise and assist leadership on matters affecting quality and improvement.
- Lead or support AHSFHC’s Population Health program efforts to ensure timely deliverables and optimal outcomes.
- Demonstrate proficiency for the skills outlined in the appropriate position skills list.
- Collaborate with QAI ream to implement patient satisfaction survey at all clinic sites.
- Lead and involve appropriate AHSFHC staff to maintain and obtain in good standing ongoing Patient Centered Medical Home (PCMH) certification.
- Plan and prepare the policies and procedures to meet the standards of care for NCQA’s Patient Center Medical Home (PCMH) accreditation.
- Achieve and maintain regulatory and voluntary standards and accreditation relating to quality systems required for support of AHSFHC business practices.
- Engage and mentor team members to improve AHSFHC QAI program outcomes.
- Other duties as assigned by supervisor.
PREFERRED QUALIFICATIONS:
1. Ability to collect, analyze, quality assure, interpret, and present statistical data; and communicate effectively orally and in writing with health care professionals and the general public.
2. Requires experience and demonstrated abilities for working in a multi-cultural setting.
3. Proficiency in Microsoft Office: Word, Excel, PowerPoint.
MINIMUM QUALIFICATIONS:
1. B.S./B.A. in social work, social/health sciences, or nursing. MPH is preferred.
2. Valid drivers’ license and insured automobile.
3. Bilingual in one Asian language or Spanish is preferred.
Pay: $65,000.00 - $70,296.21 per year
