We are seeking a dedicated and compassionate Texas Licensed Social Worker to join our team at a skilled nursing and rehabilitation facility in Dallas. Responsibilities
- Conduct psychosocial assessments for residents upon admission and as needed
- Develop and implement individualized care plans with the interdisciplinary team
- Provide emotional support and counseling to residents and families
- Assist with discharge planning and transitions of care
- Coordinate community resources and services for residents
- Advocate for resident rights and ensure compliance with state and federal regulations
- Maintain accurate and timely documentation
- Participate in care plan meetings and facility committees
- Support residents with adjustment, grief, behavioral, and family concerns
Qualifications
- Current Texas Social Worker license required (LBSW or LMSW)
- Long-term care or skilled nursing experience preferred
- Knowledge of Medicare, Medicaid, and long-term care regulations preferred
- Strong communication and interpersonal skills
- Ability to work collaboratively with residents, families, and healthcare teams
