Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team on a PRN basis, working day and night shifts. The ideal candidate will possess a strong clinical background, excellent patient care skills, and the ability to adapt to various care settings. As an LPN, you will play a vital role in delivering high-quality nursing care, supporting residents’ health needs, and ensuring compliance with all regulatory standards.
Duties
- Provide direct patient care to residents in accordance with physician orders and established nursing protocols.
- Monitor vital signs, assess patient conditions, and document findings accurately within electronic health record (EHR) systems such as Epic or Cerner.
- Administer medications, injections, IV infusions, and other treatments while adhering to safety standards and protocols.
- Assist with activities of daily living (ADLs), including hygiene, mobility, and nutritional support.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to residents' needs.
- Ensure compliance with HIPAA regulations and maintain strict confidentiality of all medical records and patient information.
- Respond promptly to emergencies or acute changes in resident conditions, utilizing skills in airway management, first aid, and critical care when necessary.
Qualifications
- Valid LPN license in the state of employment with current CPR certification.
- Prior experience working in long-term care facilities, skilled nursing facilities, or hospital settings is highly preferred.
- Demonstrated proficiency with EMR/EHR systems such as Epic or Athenahealth; experience with medical documentation is essential.
- Knowledge of medication administration protocols including injections, IV therapy, and catheterization procedures.
- Familiarity with dementia care, Alzheimer’s care, memory care strategies, and managing residents with developmental disabilities or complex medical conditions.
- Strong assessment skills combined with physiology knowledge to evaluate resident health status effectively.
- Ability to work independently as well as part of a multidisciplinary team in a fast-paced environment.
- Excellent communication skills focused on caregiver communication and client interaction while maintaining clinical confidentiality policies.
