Per Visit Rates: $20 per visit
Hourly Rate for Training and in-office Work: $17 per hour

Qualification
1. Valid CNA Certification: Current and unrestricted CNA certification in the state of practice.
2. Experience: 2 years of CNA experience, 1 year of home health experience (preferred).
3. Clinical Skills: Proficient clinical skills and knowledge of home health practices, including assisting with activities of daily living, medication management, vital sign monitoring, and infection control.
4. Documentation and Charting: Ability to accurately document patient care activities, observations, and outcomes in compliance with agency policies and regulations.
5. Communication Skills: Excellent verbal and written communication skills to effectively communicate with patients, families, healthcare providers, and interdisciplinary team members.
6. Empathy and Compassion: Demonstrated empathy, compassion, and patience in working with patients and their families during challenging times.
7. Time Management: Effective time management skills to prioritize tasks, manage caseloads, and meet deadlines in a fast-paced environment.
8. Interdisciplinary Collaboration: Ability to work collaboratively with other healthcare professionals, including nurses, therapists, social workers, and case managers, to ensure holistic and coordinated care.
9. Adaptability: Flexibility to adapt to changing patient needs, schedules, and caseloads while maintaining a high level of professionalism and quality of care.
10. Computer Skills: Proficiency in using electronic medical record (EMR) systems, documentation software, and other technology tools relevant to home health practice.
11. Patient Advocacy: Strong advocacy skills to support patients in maintaining their independence, dignity, and overall well-being.
12. Continuing Education: Commitment to ongoing professional development and staying updated on the latest advancements, best practices, and regulations in home health and patient care.
