Title: Intake Coordinator
Department: Adult Day Services
FLSA Status: Non-Exempt
Reports To: Director
Summary: Responsible for intaking in new participants; assisting participants in applying and renewing SF Paratransit; conducting outreach; managing and documenting the Child and Adult Care Food Program (CACFP) Program.
Essential Functions:
1. Handles all general inquiry calls and emails from potential participants and referral sources. Gather information from potential participants to complete a detailed intake form. Submit the Medical Treatment Plan to the participant’s medical physician and follow-up.
2. Conducts in-person and virtual presentation outreach in the community.
3. Assists the participants in applying and renewing the San Francisco Paratransit Group Van Transportation Service.
4. Assists and maintains participant’s Child and Adult Care Food Program (CACFP) eligibility annually. Maintain and input daily meal records.
5. Maintains the participant’s Adult Day Services Participant Agreement.
6. Provides administrative support to the Director, including preparation of statistical reports and office administration.
7. Performs other duties as assigned.
Qualifications:
1. High school diploma with a minimum of two years of office experience; an associate degree is preferred.
2. Basic skills and knowledge of Microsoft Office processing (Word, PowerPoint, Excel) in English and Chinese.
3. Excellent organizational, interpersonal, verbal and written communication skills.
4. Bilingual in English and Cantonese.
5. Must have TB clearance within 12 months prior to employment and maintain clearance annually thereafter.
6. Must acquire CPR and First Aid certificates within 6 months of employment and maintain current certificates thereafter.
