Provide direct patient care to hospice patients in their homes in accordance with the established plan of care

Monitor and document patient symptoms, comfort levels, and changes in condition

Administer medications and treatments as directed by the RN Case Manager or physician

Communicate effectively with patients, families, and the interdisciplinary care team regarding patient status and care goals

Provide education and emotional support to patients and families navigating end-of-life care
-
Maintain accurate and timely documentation in the EMR within required timeframes

Provide coverage in Home Health and Palliative Care settings as needed

Valid LPN license in good standing with relevant state licensing boards.

Proven experience in hospice or palliative care settings is highly preferred.

Familiarity with EMR/EHR systems such as HCHB

Knowledge of medical terminology related to geriatrics, dementia care (including Alzheimer's), infectious disease care, and pain management.

Experience working with individuals with developmental disabilities or in assisted living environments is a plus.

Strong assessment skills combined with physiology knowledge to evaluate patient needs effectively.

Excellent communication skills for triage situations and family education; basic life support (BLS) certification is necessary.
