Part-time - 3rd shift
Primary Job Duties:
- Directs and renders care to the residents within an assigned unit as leader of the unit. Makes assignments, admits, transfers, and discharges residents.
- Administers medications, charts, assists physicians, and supervises other nursing staff according to established policies and procedures.
- Reviews medication orders and medication sheets for completeness and accuracy of information, accuracy in the transcription of physician orders, and adherence to stop order policies.
- May determine employee work assignment and has the authority to make changes and revise duties as needed to meet resident needs.
- Makes daily visits to resident’s room to observe and evaluate physical and emotional status/needs of the residents during shift. Observe resident condition and notify the charge nurse or M.D. of any change or any possible need for medication or treatment.
- Provides direct resident care when needed, i.e. takes blood pressure, weights, assists with lifting and transferring of residents, turns residents, etc.
- Initiates and reviews resident care plans for appropriate resident goals, problems, approaches and revisions based on nursing needs. Completes other required documentation.
- Receives and understands resident reports from staff on previous shifts.
- Administers prescribed medication and treatments ordered by the physician. Chart the same and note effects. Note: Charge LPNs can only administer treatments that they are authorized to administer.
- Keeps the Director of Nursing informed of resident status and other related matters via written reports and verbal communication.
- Interprets and implements facility and department policies and procedure in a fair and consistent manner.
- Notifies appropriate family members or responsible parties of any change in the resident’s condition, if charge nurse.
- Completes incident reports promptly for residents and staff when necessary.
- Keeps privileged information about residents confidential according to HIPAA guidelines
- Upholds Laurel View Village’s Code of Conduct and follows Corporate Compliance guidelines per the LVV Employee Handbook.
- Converses with residents and their families regarding problems and possible solutions, progress and plan of care, and convey a genuine sense of concern.
- Requests necessary equipment and supplies as needed.
- Improves professional knowledge through professional reading, in service education and other avenues. Attends in-service education and supervisory meetings on a regular basis.
- Answers telephone inquiries in a professional manner.
- Assists in the hiring process for nursing staff and offers recommendations for hiring and/or rejection based on applicant’s qualifications. Assists in the orientation of new personnel in job duties on unit.
- Orders prescribed medications, supplies and equipment as established by facility policies and procedures.
- Follows facility complaint procedure and adjusts or suggests solutions to resolve problems.
- Orients new residents and their family members to functions of the Department.
- Forwards diet changes to Dietary Department.
- Performs other duties as necessary and appropriate, or as directed by the RN Supervisor, Department Supervisor or Administrator.
· Completes annual compliance and HIPAA training and exhibits behavior as set forth by the Code of Compliance in the performance of their duties.
· Adheres to the Organizations Policies and Procedures regarding the Compliance Program of the organization and is timely in reporting any possible concerns.
· Follows all local, state and federal regulations as they pertain to their position.
· Adheres to the Organization’s Policies and Procedures covering Privacy and Security in Compliance with HIPAA regulations.
Education and Experience Requirements:
- Must be a LPN licensed in the state of Pennsylvania.
- Must have prior experience in long term, skilled nursing, or acute care.
- Knowledge of computer systems in a health care setting
Other Requirements:
- Must deal tactfully with staff, residents, family members, visitors, and the general public.
- Must possess supervisory ability, good communication skills, and the willingness to work harmoniously with professional and non-professional staff.
- Must be caring and compassionate to residents, families, and coworkers.
Working Conditions:
- Stands, sits, stoops, bends and walks frequently.
- Communicates verbally and in writing.
- Responds to resident calls for assistance.
- May be required to work after regular working hours, on holidays and on weekends.
