The City of Lebanon is looking for an experienced professional to join the Fire Department as a Mobile Integrated Healthcare Provider. The position is a professional public safety and healthcare position responsible for providing Mobile Integrated Health (MIH) and community paramedicine services to individuals with complex medical, behavioral health, social, and healthcare access needs. The position works collaboratively with the LFD Community Nurse to conduct in-home and community-based assessments, provide care within approved protocols and scope of practice, coordinate services with healthcare and community partners, support high-risk patients following hospital discharge, and reduce preventable emergency department visits and EMS utilization. Additionally, this position is responsible for operating as a Firefighter/Paramedic during normal business hours as needed.
Responsibilities include, but are not limited to:
- Conducts scheduled and unscheduled community paramedicine visits in homes and other community settings, performing patient assessments within the individual’s scope of practice and approved MIH protocols.
- Develops and supports individualized care plans in collaboration with the LFD Community Nurse, patients, caregivers, primary care providers, hospitals, home health agencies, behavioral health providers, and other members of the healthcare team.
- Provides patient education regarding medications, chronic disease management, injury and illness prevention, appropriate use of emergency services, and available healthcare and community resources.
- Performs post-discharge and high-risk patient follow-up, including assessment of changes in condition, medication reconciliation support, vital-sign monitoring, and identification of barriers to successful recovery or ongoing care.
- Identifies unmet medical, behavioral health, social, transportation, housing, food, safety, and other needs that may affect patient health; assists patients with referrals and connections to appropriate community resources and services.
- Prepares accurate and timely patient care reports, referral documentation, care coordination notes, program activity records, and other reports as required.
- Responsible for entering program and patient care data into approved electronic patient care reporting, healthcare, and City information systems while maintaining patient confidentiality.
- Performs approved point-of-care assessments, monitoring, medication-related assistance, and other community paramedicine interventions consistent with New Hampshire EMS scope of practice, medical direction, departmental policy, and program protocols.
- Maintains current knowledge of Mobile Integrated Health, community paramedicine, EMS clinical practice, chronic disease management, behavioral health, harm reduction, social determinants of health, and available community resources.
- Responds to fires, EMS incidents, and/or other major emergencies during normal business hours at the discretion of the shift or chief officer. Receives supervision through the chain of command or the Incident Command System. If the Community Paramedic is the senior member on scene, will assume command, evaluate the situation, perform an initial size-up, make operational decisions for controlling the emergency, direct apparatus, assign personnel and equipment as needed, and continue until relieved by a more senior member. Reads, assimilates, interprets, and acts upon information contained in pre-plan books, hazardous materials manuals, EMS protocols, and other technical resources as required.
- Works collaboratively with the LFD Community Nurse to coordinate care and referrals with hospitals, primary care practices, home health agencies, behavioral health and substance use treatment providers, social service agencies, public health organizations, senior services, and other community partners.
- Participates in case conferences, multidisciplinary care coordination meetings, quality improvement activities, and program development initiatives related to Mobile Integrated Health and community paramedicine.
- Conducts outreach to patients with frequent EMS utilization or identified healthcare access needs and assists in developing strategies to improve continuity of care and appropriate use of available services.
- Able to drive and operate all LFD motorized fire apparatus. Able to operate and identify safety/operational parameters for all LFD equipment.
- Functions at the minimum EMS certification of New Hampshire Paramedic, performing emergency and community-based medical services as directed or required, and completing continuing education and training required to maintain certification.
- Performs other related duties as required.
Other Functions
- Maintains current knowledge of applicable New Hampshire EMS laws, rules, protocols and scope of practice; patient privacy requirements; departmental policies; medical direction; community paramedicine and Mobile Integrated Health standards; and local healthcare and social service resources.
- Submits a monthly summary of Mobile Integrated Health/Community Paramedicine activities to the Lebanon Fire Chief.
Minimum Qualifications:
Education, Training, and Experience:
- Associate’s degree in paramedicine, emergency medical services, healthcare, public health, or a related field is preferred; an equivalent combination of education, training, and experience may be considered.
- At least three (3) to five (5) years of progressively responsible EMS experience, preferably as a paramedic. Experience with community paramedicine, Mobile Integrated Health, care coordination, chronic disease management, behavioral health, or community-based healthcare is preferred.
- Must have current CPAT certification at the time of hire.
- Must be able to successfully complete pre-employment physical.
Licenses and Certifications
- Must possess or be able to acquire within three (3) months of date of hire, a valid driver’s license to operate motorized fire apparatus.
- Must be able to complete New Hampshire Certified Firefighter Level II within one (1) year from date of hire.
- Must possess and maintain current New Hampshire Paramedic certification or licensure and meet all requirements necessary to function as a paramedic for the Lebanon Fire Department.
