PRN Hospice Social Worker (LMSW/MSW) – San Antonio
Vara Hospice is growing in San Antonio and we’re looking for a compassionate, dependable PRN Social Worker to join our hospice team.
This is a flexible position for someone who enjoys meaningful work with patients and families during one of life’s most important transitions. Our San Antonio census is growing, so this role is ideal for an experienced hospice or healthcare social worker looking for flexible PRN hours with room to grow.
What you’ll do
- Complete psychosocial assessments and follow-up visits
- Support patients and families through end-of-life care
- Assist with community resources, advance care planning, and caregiver needs
- Participate in interdisciplinary team care and care planning
- Document visits timely in the EMR
- Coordinate closely with nurses, providers, chaplain, and hospice leadership
What we’re looking for
- Current Texas social work license
- MSW/LMSW preferred
- Healthcare social work experience required
- Hospice or home health experience strongly preferred
- Reliable transportation
- Compassionate, organized, and comfortable working independently
- Bilingual English/Spanish is a plus
