Responsibilities
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to ensure comfort and dignity.
- Monitor patients’ health by taking vital signs, observing for changes in condition, and reporting findings accurately to healthcare professionals.
- Support medication administration following prescribed care plans while adhering to HIPAA regulations to maintain confidentiality.
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary.
- Use specialized equipment like Hoyer lifts or resident’s lifts safely to assist with mobility and prevent injury.
- Maintain a clean and safe environment by performing light cleaning duties and adhering to infection control protocols.
- Collaborate with healthcare teams to develop and implement personalized care plans that address each individual’s unique needs.
Skills
- Proven experience in home health, assisted living, nursing homes, or group homes with a strong understanding of patient observation and monitoring.
- Knowledge of Alzheimer’s care, dementia care, memory care, autism support, and hospice care practices.
- Certifications such as First Aid, CPR, HIPAA compliance training, and medication administration are highly preferred.
- Ability to lift heavy objects safely and perform physically demanding tasks while maintaining patient safety.
- Excellent communication skills for working effectively with individuals with disabilities or developmental disabilities.
- Familiarity with working within care plans and documenting patient progress accurately.
- Compassionate attitude combined with strong organizational skills to manage multiple responsibilities efficiently.
