Responsibilities:
Evaluate and treat residents requiring physical therapy
Develop and implement individualized treatment plans
Set functional, realistic goals for patients
Complete documentation (evaluations, progress notes, discharges)
Collaborate with nursing and interdisciplinary team
Educate patients and families
Qualifications:
Graduate of an accredited Physical Therapy program
Active Maryland PT license
SNF/long-term care experience preferred (not required)
