Responsibilities
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting to promote dignity and independence
- Monitor patient health by observing vital signs, recognizing changes in condition, and reporting findings accurately
- Following care plans and HIPAA regulations to ensure safety and compliance
- Support clients with mobility using proper patient transfer methods such as Hoyer lifts and manual handling techniques
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary
- Provide companionship and emotional support to enhance mental well-being, especially for those with dementia or Alzheimer's disease
- Maintain a clean, safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing living spaces
- Assist with transportation for medical appointments or community activities, ensuring safe patient transportation assistance
- Document care activities accurately in client records and communicate effectively with healthcare teams about patient status
- Support clients with specialized needs including autism spectrum disorder, neurodevelopmental disorders, or developmental disabilities through personalized care strategies
Skills
- Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments
- Strong patient observation skills to detect changes in condition or behavior promptly
- Knowledge of dementia care, memory care, hospice care, and Alzheimer’s support techniques
- Familiarity with care plans, medication administration, vital signs monitoring, and patient personal care assistance
- Ability to perform CPR and first aid confidently; manual handling and heavy lifting capabilities required
- Experience working with individuals with intellectual disabilities or developmental disabilities support needs
- Excellent client communication skills coupled with compassionate caregiving practices
- Proficiency in HIPAA compliance to ensure confidentiality of patient information
- Skilled in assisting with ADLs using appropriate patient transfer methods such as Hoyer lifts or other mobility aids
- Home & community care experience including meal preparation, personal transportation assistance, and home safety assessments
- Knowledge of geriatrics principles and long-term care best practices for senior patients
