Provide initial and ongoing assessment to determine level of functioning.

Responsible in gathering background data, reviews referrals, selects and modifies evaluation tools, and evaluates patients determining the level of patient's physical and psychosocial functioning.

Responsible in developing and revising the plan of care in consultation with the physician and other careteam members.

Responsible for planning, evaluation and treatment implementation of assigned caseload.

Responsible in preparing clinical progress notes.

Responsible in supervising therapy assistants and home health aides as appropriate.

Responsible for evaluating outcomes.

Adhere to all Company policies and procedures.

Minimum education (or substitute experience) required: Currently licensed by the California Board of Occupational Therapy.

Minimum experience required: Minimum of one year in acute care setting. Two years experience in Home Health preferred.
