Mount Sinai New York Concierge Care in Palm Beach is seeking an exceptional, service-oriented Licensed Practical Nurse (LPN) or Certified Medical Assistant (CMA) to join our concierge practice.
Responsibilities include:
- Prepare patient for office visit or telemedicine appointment. Obtain vital signs and EKG when appropriate. Document all pertinent information in electronic medical record (medications, allergies, patient history, etc.). Complete medication refill requests and other tasks in a timely manner.
- Administer medications, vaccines, injections, point-of-care testing, and specimen collection within scope of practice.
- Send and receive medical records, obtain reports, hospital notes, referral information, etc. via fax and electronic exchange. Complete forms and requisitions. Scan and manage patient records to ensure information is complete and categorized appropriately.
- May coordinate referrals, specialist appointments, diagnostic testing, imaging, and follow-up care in conjunction with the Patient Navigator
- Track outstanding orders, test results, and preventative health screenings to ensure timely completion.
- Collaborate with internal departments and external healthcare providers to facilitate seamless transitions to care.
- Support same-day appointments and accommodate urgent patient needs whenever possible.
