Join a nation-wide primary care organization focused on providing exceptional care to the geriatric population. We are seeking a Spanish-speaking Primary Care Advanced Practice Provider for our Brooklyn location. This is a full-time, permanent position with a comprehensive benefits package.
- Provide primary care services to a patient population primarily consisting of Medicare recipients (over age 65), with some Medicaid patients under age 65.
- Work collaboratively with a care team consisting of a physician, advanced practice provider, scribe, medical assistant, and shared RN/social work support.
- Utilize specialty support from a partnered telemedicine company for quick consults with specialists such as dermatologists, cardiologists, and gastroenterologists.
- Utilize EPIC EMR, with scribe support, to manage patient records and care plans.
- Participate in a call schedule rotated amongst the market, with calls being limited and 100% remote.
- Monday to Friday, 8am-5pm
- No nights or weekends
- 12-16 patients per day once fully ramped up

- Previous primary care or skilled nursing facility/nursing home experience preferred, but not required
