Job Summary
The Registered Nurse (RN) Supervisor is responsible for providing essential clinical oversight, staff training, and regulatory compliance for Personal Care Services (PCS) delivered by Personal Care Workers (PCWs) within a Wisconsin Medicaid-certified Personal Care Agency. This position ensures all services are provided according to the recipient's written plan of care (POC), physician orders, and the requirements of the Wisconsin Department of Health Services (DHS 105.17 and DHS 107.112). The RN Supervisor works collaboratively with the agency administrator, personal care workers, recipients, and relevant managed care organizations (MCOs) or state agencies to safeguard the health, safety, and independence of participants.
Responsibilities
Assessment and Care Planning:
- Conduct initial nursing assessments and ongoing evaluations of all personal care recipients to determine the need for services.
- Secure and interpret written physician orders for personal care services, ensuring orders are renewed at least every 3 months or as the client's needs change.
- Develop and implement a written Plan of Care (POC) for each recipient, including service frequency, assigned tasks, and giving full consideration to the recipient's preferences.
- Review and update the POC and evaluate the recipient's condition at least every 60 days, including a visit to the recipient's home.
Supervision and Quality Assurance:
- Supervise the provision of personal care services by Personal Care Workers (PCWs), as required by s. DHS 107.112(3)(a) and (c).
- Perform a supervisory visit to the recipient's home at least every 60 days, which must include direct observation of the PCW providing care.
- Provide written instructions and demonstrate to the PCW how to perform the assigned tasks.
- Evaluate the competency of PCWs to perform assigned tasks and ensure they are trained in each assigned skill, documenting a successful demonstration.
- Review the PCW's daily written record of services provided and report any significant changes in the recipient's condition to the physician.
- Ensure all services are documented and provided in alignment with authorized physician orders and the Plan of Care.
Personnel and Regulatory Compliance:
- Oversee the orientation and ongoing instruction of PCWs in compliance with agency policy and administrative rules.
- Evaluate every PCW for performance and adherence to agency policies and DHS 105.17 and DHS 107.112.
- Monitor and enforce adherence to infection control practices, incident reporting, and all state health and safety regulations.
- Ensure all staff comply with recipient rights, confidentiality/HIPAA standards, and agency grievance procedures.
- Maintain all required records, including nursing assessments, physician orders, the Plan of Care, and RN supervisory visit documentation, for state audits.
- Report suspected abuse, neglect, or exploitation in accordance with applicable Wisconsin statutes.
Qualifications
- Must be a Registered Nurse (RN) currently licensed and in good standing with the Wisconsin Board of Nursing
- Required Experience: Must have training and experience in the provision of personal care services or a related program, and at least two (2) years of supervisory or administrative experience in personal care services or a related health program
- Thorough knowledge of Wisconsin Administrative Code DHS 105.17 (Personal Care Providers) and DHS 107.112 (Personal Care Services), including documentation and supervision requirements.
- Strong clinical assessment and supervisory skills, with the ability to guide and evaluate non-clinical staff (PCWs) providing in-home care.
- Excellent communication, organizational, and documentation skills; able to collaborate effectively with recipients, families, physicians, and case management/MCO staff.
- Must possess a valid driver’s license, reliable transportation, and the ability to travel locally for in-home visits and staff supervision.
