Duties
- Deliver comprehensive patient personal care assistance, including help with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support using patient transfer methods like Hoyer lifts and manual handling techniques.
- Monitor vital signs, observe patient conditions, and report changes promptly to ensure safety and well-being.
- Administer medications following care plans while adhering to HIPAA regulations and safety protocols.
- Assist with meal preparation, feeding, and nutritional needs to promote health and comfort.
- Support patients with cognitive challenges such as dementia or Alzheimer's by implementing specialized memory care strategies and engaging in meaningful activities.
- Maintain accurate documentation of patient progress, care plans, medication administration, and incident reports.
- Provide transportation assistance for appointments or community outings while ensuring safety during transfers and mobility support.
Experience
- Prior experience working in group homes, nursing homes, assisted living facilities, or home health environments is highly preferred.
- Demonstrated expertise in patient observation, monitoring vital signs, medication administration, and implementing care plans.
- Knowledge of dementia care practices such as Alzheimer’s and memory care is essential for providing compassionate support.
- Familiarity with HIPAA compliance, patient monitoring tools, first aid & CPR certification is required to ensure safety standards are met.
- Hands-on experience using equipment like Hoyer lifts or patient transfer methods enhances your ability to assist safely during manual handling tasks.
- Strong communication skills for client interaction and team collaboration are vital to delivering consistent quality care.
