Clinical Manager – Home Health Agency

Sage in the Ranch Home Health Agency is a compassionate and growing home health organization dedicated to providing high-quality, patient-centered care. We are looking for a reliable and experienced Clinical Manager (RN) to join our leadership team and oversee clinical operations, ensure regulatory compliance, and support our field staff in delivering exceptional home health services.
Responsibilities:
- Oversee and coordinate clinical operations of the home health agency.
- Supervise and support field nurses, therapists, and caregivers.
- Review and approve clinical documentation, ensuring accuracy and compliance with state and federal regulations.
- Develop and implement care plans that meet each patient’s unique needs.
- Participate in case conferences and quality assurance activities.
- Provide ongoing staff education, training, and performance evaluations.
- Collaborate with physicians, patients, and family members to ensure quality outcomes.
- Ensure compliance with all Medicare, state, and agency policies.
Qualifications:
- Registered Nurse (RN) with an active California license (required).
- Minimum 2 years of experience in home health or a similar clinical management role.
- Strong leadership, communication, and organizational skills.
- Knowledge of Medicare regulations, OASIS documentation, and home health standards.
- Ability to multitask and work efficiently in a fast-paced environment.
- Reliable transportation and valid driver’s license.
