Duties
- Assess patients’ physical conditions through comprehensive evaluations, including physiology and anatomy knowledge, to inform effective treatment strategies.
- Develop and implement individualized care plans tailored to each patient’s needs, focusing on rehabilitation goals for orthopedics, neurology, pediatrics, geriatrics, and other specialties.
- Administer therapeutic interventions such as trigger point therapy, physiotherapy techniques, and patient education to facilitate recovery and improve functional outcomes.
- Monitor patient progress regularly, adjusting treatment plans accordingly while maintaining detailed clinical documentation that adheres to standards for medical terminology and electronic health records (EHR) management.
- Collaborate closely with healthcare professionals—including physicians, nurses, occupational therapists—and communicate clearly with patients to ensure comprehensive care delivery.
- Manage discharge planning processes by coordinating follow-up care and providing guidance for home & community care settings.
- Ensure compliance with clinical documentation standards and participate in quality assurance activities related to outpatient rehabilitation center experience and inpatient hospital settings.
Requirements
- Valid licensure as a Physical Therapist in the respective jurisdiction with current practicing credentials.
- Proven experience in outpatient clinic settings or inpatient hospital environments, demonstrating proficiency in acute care and outpatient rehabilitation.
- Strong knowledge of physiology, anatomy, medical terminology, and treatment modalities such as trigger point therapy.
- Experience working with diverse patient populations including geriatrics, pediatrics, neurology cases, and those requiring home health services.
- Familiarity with outpatient experience, EMR (Electronic Medical Records) systems or EMR systems for documentation purposes, along with adherence to clinical documentation standards.
- Ability to develop effective treatment plans based on thorough assessments and collaborate effectively within multidisciplinary teams.
- Excellent client communication skills coupled with a compassionate approach to patient care across community care settings.
