JOB SUMMARY: The Admissions Assessor is responsible for reviewing referrals to the nursing home and making decisions on acceptance for admission based on budgeted occupancy, payer mix and clinical criteria.
DUTIES AND RESPONSIBILITIES:
- Monitors referrals for admission from electronic and other sources, and makes timely decision for acceptance for admission.
- Ensures clinical appropriateness of new admissions, through consultation with nursing, medicine, rehabilitation and/or pharmacy as needed.
- Ensures that new admission's source of payment is validated, through checks in Omnipro, verification of prior stays, managed care authorization, and/or upon consultation with Finance as needed.
- For all managed care referrals, ensures that authorization is received prior to admission, and that this information is entered into the system.
- Ensures that a current PRI is received and scanned into the system for each new referral.
- Assists with tours to interested patients/residents and families as needed.
- Collaborates with Community Outreach Coordinator on seeing potential referrals and for patients/residents who have been hospitalized.
- Conducts field visits as needed (to hospitals or other referral sources) to evaluate and facilitate admissions where appropriate.
- Promotes, supports and delivers excellent customer service to all internal and external customers.
- Adheres to Human Resources policies and procedures, including facility and departmental orientation and mandatory training and education.
- Provides coverage for weekend admissions when necessary.
- All other assigned duties.
JOB QUALIFICATIONS
- Bachelor's degree in health or social sciences field.
- 3 years of experience working in an admissions department or as a healthcare professional.
- Prior experience of working in skilling nursing or other long-term care program preferred.
Compensation: $30.2197/hr - $39.5604/hr
