Position Summary: RN or LVN Charge Nurse
As a Charge Nurse (RN or LVN), you will play a vital role in providing high-quality care and promoting the well-being of each resident based on their individual care plans and assessments. This position is ideal for experienced, passionate nurses who want to make a meaningful difference in a skilled nursing facility setting.
Key Responsibilities:
- Monitor, assess, and document resident conditions during rounds.
- Implement and follow care plans as directed.
- Administer prescribed medications accurately.
- Participate in physician rounds and chart visits.
- Perform narcotic reconciliation between shifts.
- Identify significant changes in resident health and take appropriate action.
- Oversee and support nursing aides during your shift.
- Complete incident and accident reports as per policy.
- Conduct resident discharges in accordance with procedures.
- Complete nursing assessments before contacting physicians.
- Communicate clearly with residents and their families regarding health updates.
- Perform other tasks as assigned by the Director of Nursing or Administrator.
Qualifications:
Education:
- Graduate of an accredited LVN or RN program.
Licensure:
- Active LVN or RN license in good standing in the state of Texas.
Experience:
- Previous experience in a skilled nursing facility or long-term care setting preferred.
