Job Title: Direct Support Professional (DSP)
Location: Services provided in the client's home or community settings. The client’s home is in Mankato, Minnesota
Compensation: $18.00 – $20.00 per hour, depending on experience and education.
Schedule: 30 hours per week, 5 days per week (Monday-Friday), and 12 per week (Saturday and Sunday.
Job Summary:
We are seeking a compassionate, reliable, and dedicated caregiver to provide Integrated Community Supports (ICS) service to an individual with disabilities in their home or community setting. The ideal candidate will support clients in achieving greater independence, health, and quality of life.
Key Responsibilities:
1. Individualized Skill Development & Support:
- Assist individuals in developing and maintaining:
- Community living skills, including navigating public transportation, using community resources, and social engagement.
- Health and wellness routines, such as medication reminders, attending medical appointments, and promoting healthy eating and hygiene.
- Household management, including budgeting, cleaning, laundry, cooking, and organizing personal belongings.
- Self-advocacy and decision-making skills that encourage independence.
- Provide prompts, cues, modeling, or hands-on assistance as needed based on the person’s support plan.
2. Implementation of Support Plans:
- Deliver services based on the individual’s person-centered support plan and coordinated service and support plan (CSSP).
- Collaborate with case managers, guardians, and interdisciplinary teams to ensure continuity and consistency of services.
- Participate in plan reviews and provide feedback to update goals and support strategies as needed.
3. Health, Safety & Crisis Prevention:
- Monitor and report changes in health, behavior, or environment that may affect the individual’s safety and well-being.
- Implement and follow positive behavior support strategies, de-escalation techniques, and emergency protocols.
- Ensure safe administration of medications (if delegated) and proper use of medical equipment or assistive technology.
- Conduct regular health and safety checks of the living environment to meet licensing and program standards.
4. Documentation & Compliance:
- Maintain daily progress notes that accurately describe services provided, the individual’s response, and any notable incidents.
- Document data as required for service billing (e.g., time-in/time-out, service type).
- Assist with the completion of reports, incident forms, and mandated documentation per DHS and company policies.
- Maintain confidentiality and uphold HIPAA standards at all times.
5. Community Engagement:
- Facilitate participation in social, recreational, and educational activities that reflect the individual’s interests and goals.
- Support individuals in developing and maintaining natural relationships and social connections.
- Encourage exploration of employment, volunteer, or educational opportunities when appropriate.
6. Communication & Team Collaboration:
- Communicate effectively with individuals, families, guardians, and team members to support coordinated care.
- Attend staff meetings, training sessions, and case conferences as required.
- Report concerns or changes in the individual’s condition promptly to supervisors or healthcare providers.
Qualifications:
- Must be 18 years or older.
- High school diploma or GED preferred.
- Experience working with individuals with disabilities or vulnerable adults is strongly preferred.
- Ability to complete the required training (including PCA/CFSS Worker Training, and 245D Orientation Training).
- Pass a background check per Minnesota Department of Human Services (DHS) guidelines.
- Ability to communicate effectively, both verbally and in writing.
- Reliable transportation may be required depending on the service location.
