Greenwood House Hospice
Position Summary
Greenwood House Hospice is seeking a compassionate, relationship-focused Licensed Practical Nurse (LPN) to join our team as a Clinical Liaison. This role serves as a key connection between healthcare providers, referral sources, patients, families, and our hospice care team. The Clinical Liaison is responsible for educating healthcare professionals and community partners about hospice services, facilitating referrals, and ensuring a smooth transition to hospice care for eligible patients.
Essential Responsibilities
- Develop and maintain strong relationships with hospitals, skilled nursing facilities, assisted living communities, physician offices, and other referral sources.
- Educate healthcare professionals, patients, and families about hospice philosophy, eligibility criteria, and available services.
- Conduct patient assessments and assist in determining hospice appropriateness in collaboration with the interdisciplinary team.
- Coordinate referrals and admissions while ensuring compliance with regulatory requirements.
- Serve as a resource for patients, families, and healthcare partners throughout the referral and admission process.
- Participate in community outreach activities, healthcare events, and educational programs.
- Maintain accurate referral documentation and timely communication with referral sources.
- Collaborate with hospice leadership to support census growth and service excellence.
- Ensure exceptional customer service and responsiveness to referral partners.
Qualifications
- Current Licensed Practical Nurse (LPN) license in good standing.
- Minimum of 2 years of clinical nursing experience required.
- Hospice, home health, palliative care, case management, or care coordination experience preferred.
- Strong communication, presentation, and relationship-building skills.
- Ability to work independently and manage multiple priorities.
- Knowledge of hospice regulations, eligibility guidelines, and end-of-life care principles preferred.
- Valid driver's license and reliable transportation for local travel.
