Job Overview
We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will play a crucial role in ensuring the safe and effective use of medications for patients. This position requires a strong background in clinical pharmacy, patient care, and the ability to work collaboratively within a multidisciplinary team. The Pharmacist will be responsible for medication management, patient assessments, and providing exceptional patient service in a dynamic healthcare environment.
Responsibilities
- Review and interpret medication orders to ensure accuracy and safety.
- Provide clinical pharmacy services including patient assessment and medication therapy management.
- Collaborate with healthcare professionals to optimize patient care plans.
- Monitor patient responses to medications, adjusting therapies as necessary.
- Educate patients and healthcare staff on medication use, side effects, and interactions.
- Prepare and dispense medications, including IV infusions, adhering to aseptic techniques.
- Maintain accurate records of medication administration and patient care activities.
- Participate in hospital committees related to pharmacy services and medication safety.
- Stay current with medical terminology, physiology, anatomy knowledge, and advancements in pharmacy practice.
Skills
- Proficient in managed care principles and practices.
- Experience in Level I or Level II trauma centers is preferred.
- Strong background in hospital medicine and critical care experience.
- Completion of a pharmacy residency program is highly desirable.
- Clinical pharmacy experience with a focus on acute care settings such as ICU or PICU.
- Knowledge of pain management strategies and acute pain management protocols.
- Familiarity with emergency room operations and medication administration processes.
- Excellent communication skills for effective patient service and collaboration with healthcare teams.
