Overview
We are seeking a dedicated and compassionate Licensed Vocational Nurse to join our team. Vitality Care Facility is an Adult Residential Facility in Sacramento, CA. We take care and support four individuals who call this place home. Vitality Care Facility supports individuals who may need 24 hour nursing care and supervision due to their developmental / intellectual disabilities, as well as health and behavioral issues. The Licensed Vocational Nurse will be working alongside Direct Support Professionals during the shift in order to meet all of the needs of the clients.
Responsibilities
- Assess and monitor the individuals' health and promptly reporting to MD changes from baseline.
- Administer medications and treatments as prescribed, ensuring adherence to standards of care.
- Provide nursing care within scope of practice.
- Collaborate with a multidisciplinary team to develop and implement individualized care plans for patients.
- Educate clients and staff on health management and how to achieve client's health goals
- Maintain accurate client healthcare and behavioral records (Medication Administration, Seizure log, Weight, Input / Output, etc
- Qualifications
- Current LVN license in California
- Pass background check - Fingerprint Background check through Live Scan service required.
- Experience working with developmentally disabled / intellectually disabled clients preferred.
- Strong communication skills with the ability to work effectively in a team-oriented environment.
- Compassionate demeanor with a commitment to providing exceptional patient-centered care and an advocate for client's rights

Available shifts: 12hour shifts per day (7a & 7p), block shifts of Monday-Wednesday, Thurs-Saturday. Every other Sunday shift available.
