Job Title: Behavioral Healthcare Manager (Full-Time)

Seniority Healthcare is seeking a dedicated and compassionate Behavioral Healthcare Manager to join our collaborative and innovative team. This role is key in delivering mental health care within a Senior Living Care Community environment, working alongside Seniority Healthcare’s Primary Care Team and Staff Psychiatrist. Key Responsibilities:
- Collaborative Care: As part of the CMS Collaborative Care Model, coordinate and manage mental health care for senior patients with conditions such as depression, anxiety, and dementia in close collaboration with the primary care team, staff psychiatrist, and community caregivers.
- Patient-Centered Care: Develop and implement care plans that address the unique behavioral health needs of geriatric patients, ensuring seamless integration of psychiatric and primary care services.
- Care Coordination: Act as a liaison between healthcare providers, patients, and families to ensure efficient care delivery and clear communication.
- Monitoring & Reporting: Regularly monitor patient progress, adjusting care plans based on outcomes, and providing updates to the healthcare team.
- Team-Oriented Approach: Collaborate closely with senior living community staff, residents’ families, and our healthcare team to create a compassionate, supportive, and effective care environment.
- Travel: Routine travel between senior living communities will be required.
Qualifications:
- Bachelor's or Master’s Degree in Social Work, Psychology, Nursing, or a related field.
- Minimum of 3 years of experience in behavioral health management, preferably working with seniors or in a primary care setting.
- Strong knowledge of the CMS Collaborative Care Model and experience in integrating behavioral health services with primary care.
- Excellent communication, organization, and interpersonal skills.
- Ability to work autonomously and as part of a cohesive, multidisciplinary team.
- Flexibility and willingness to travel to multiple locations as needed.
