We are the largest private provider of residential and community-based services for adults with intellectual and developmental disabilities (IDD) in the state of Kentucky. We are a local non-profit organization founded in 1970 by Louisville-area parents of people with IDD. We believe that a well-trained staff, committed to the mission and vision of Cedar Lake, will successfully offer a high level of compassionate, capable care, responsive to the needs of the mind, body, and the spirit of the people we serve. What you’ll do:
Family Home Providers are contracted through Cedar Lake to provide ongoing care for individuals with IDD. The participant lives in the provider’s home, has a private bedroom, and access to communal living spaces. FHPs help individuals experience everyday life—shopping, dining out, vacations, and more—while fostering independence and inclusion.

Provide person-centered supports, encouraging each individual’s highest practical physical, mental, and psychosocial well-being.

Foster a safe, caring, and respectful home environment.

Support individual goals to increase self-reliance and independence.

Maintain knowledge of the individual’s care plan and implement team-developed strategies.

Observe and report changes in condition or behavior promptly.

Complete required daily documentation reflecting progress and concerns.

Maintain privacy, dignity, and rights of the individual, including freedom from abuse and restraints.

Immediately report any incidents or evidence of abuse, neglect, exploitation, or rights violations.

Assist with daily living skills and personal hygiene (bathing, dressing, grooming, feeding, toileting, etc.)

Administer medications per protocol and maintain accurate records.

Provide transportation to appointments and community activities.

Support personal financial responsibilities and needs.

Maintain the home environment, including housekeeping and scheduling.

Encourage community participation and facilitate friendships.

Guide on social, judgment, health, and safety issues.

Attend required trainings and meetings.

Protect and assist with care of personal belongings.

Communicate effectively with individuals, families, and Cedar Lake staff.

Perform other duties as assigned.
What sets you apart:

High School Diploma or equivalent is required.

Experience supporting Individuals with Intellectual and/or Developmental Disabilities is preferred.

Valid driver’s license is required.
Contract Details:

FHPs are independent contractors, not employees. Monthly home reviews and ongoing support provided. Relief network available through Co-FHP program.
