Case Manager – Licensed Nurse LVN

Essential Functions
- Provide case management services to post-acute care residents in our facility.
- Develop preliminary and comprehensive assessments of the needs of each resident.
- Will work on management team with other department managers to ensure collaboration between departments.
- Collaborate with residents, families, staff, and managed care groups to develop a comprehensive resident care program.
- Make rounds to assess quality of care performance, assuring quality service.
- Admit, transfer, and discharge residents as required.
- Review nurses' notes to determine if the care plan is being followed. Discuss problem areas with the director of Nursing Services.
- Ensure that the care plan identifies any special equipment and utensils the resident may need (e.g., plate guard, enlarged silverware handles, etc.)
- Communicate changes in resident’s condition.
- Develop, maintain and secure records, reports, instructional manuals, and reference materials pertinent to in-service program.
- Adheres to and assures compliance with Code of Conduct, facility policies and procedures and all applicable rules, regulations and standards as promulgated by Federal, State, and accrediting agencies or regulating bodies. This includes, but is not limited to, Department of Health, Centers of Medicare and Medicaid Services, and other applicable regulatory agencies.

Education / Requirements
- Nursing Degree from an accredited college or university, or be a graduate of an approved LVN or RN Program.
- 3 more years experience in a supervisory capacity in a hospital, skilled nursing care facility, or other related medical facility.
- Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
- Must be able to read, write, speak, and understand the English language.
- Current CPR certification
- Must pass pre-employment back ground check and physical
