Housing Stability Case Manager
Position Summary
The Housing Stability Case Manager provides individualized, person-centered case management to residents of Jennifer House, a supportive living residence for individuals reentering the community after incarceration or other justice-involvement. This role leads assessments, individual service planning, housing determinations, and documentation, helping each resident build toward stable, sustainable housing.
Key Responsibilities
- Conducts individualized assessments and develops, implements, and updates Individual Service Plans (ISPs)
- Leads housing search, placement, stabilization, and retention planning for each resident
- Coordinates referrals to behavioral health, medical care, employment, education, benefits, and transportation
- Meets individually with each resident weekly for 1:1 case management sessions
- Maintains accurate, timely HMIS documentation and related paperwork
- Establishes measurable goals and monitors resident progress toward housing stability
- Plans and facilitates group activities and open office hours for residents
- Participates in admissions and intake alongside the Director of Jennifer House
Reporting Relationship
Reports to the Director of Jennifer House.
Compensation
$55,000–$60,000, dependent on experience. 30–40 hours per week, dependent on experience and candidate preference; some evening availability expected (approximately once per week).
Qualifications
- Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Criminal Justice, or a related field preferred
- Equivalent combinations of education, professional experience, case management experience, and lived experience will be considered
- Experience in supportive housing, case management, reentry, or a related field preferred
- Knowledge of assessments, service planning, Housing First, trauma-informed care, and HMIS
- Strong documentation, communication, organization, and problem-solving skills
- This position is not open to current residents of Spiritus Christi Prison Outreach programs, or to alumni who completed a program within the past five years.
