This is a full time position located in our Rockwall Office
Previous Home Health Case Manager experience is required for this position.
Consulting with the physician (attending) in establishing and altering plans of treatment.
Participating in all quality improvement activities of the agency.
Enforcing personnel policies and reporting all personnel matters requiring action to the Director of Nurses.
Report directly to Director of Nurses regarding problems with the admission of patients or continuation of care.
Responsible for ordering equipment for patients and coordinating the pick-up of equipment when the patient is discharged.
Responsible for seeing that IDT meetings are conducted with appropriate personnel.
Participates in the after hours on-call rotation; including holiday and weekends.
Striving for self-improvement through attendance at in-services, workshops, conferences and professional organizational meetings.
Responsible for assistance with in-service programs for home health aides as needed.
Responsible for keeping all physicians’ orders current.
Coordinating services and delegating responsibilities.
Evaluating the quality of care given to patients and making revisions and modifications when necessary.
Admission of new patient, Recertifications, Discharges, ROC, SCIC, Transfers and home visits to problem patients as needed.
Giving appropriate reports to designated individuals in a timely manner.
Assisting personnel in developing various skills relating to patient needs.
Participating in staff development activities.
Assuming additional duties if indicated.
Participating in the orientation of new staff.
