RN Case Manager Home Health Qualifications:
- Current RN license in good standing
- Minimum of 1 year nursing experience
- Preferred Home Health experience and comprehensive knowledge of OASIS but not required
- Strong clinical assessment and case management skills
- Must be able to complete documentation within 24 hours
- Demonstrated success working independently while taking on increased responsibilities
- Excellent organization, interpersonal and communication skills
- Prior experience with electronic medical record documentation and reporting systems
RN Case Manager Home Health Responsibilities:
- Direct and coordinate the care provided by a multi-disciplinary care team
- Deliver exceptional client service and quality clinical care
- Ensure adherence to policies and procedures
- Complete Comprehensive assessments, provide direct patient care, client and caregiver teaching
- Review clinical documentation, including OASIS and care plan coding
- Act as internal case manager and client advocate
- Support the medical home model
- Travel required - limited call required
