What You’ll Do
- Conduct thorough patient assessments and create individualized care plans.
- Coordinate and oversee patient services, ensuring compassionate, quality care.
- Educate and support patients and families on health management and discharge planning.
- Collaborate with an interdisciplinary team to optimize outcomes.
- Monitor progress, update care plans, and adjust to changing needs.
What We’re Looking For
- Current RN License (in good standing).
- Experience in hospital, critical care, or discharge planning preferred.
- Strong knowledge of patient assessment, care coordination, and managed care.
- Familiarity with geriatrics and post-acute care.
- Excellent communication and patient relationship skills.
