Lehigh Human Support Services is seeking a compassionate and dependable Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide flexible, part-time nursing support within our Chapter 6400 Licensed Residential Community Living Arrangement (CLA) Homes. This role supports the health, safety, and well-being of individuals with intellectual and developmental disabilities while working collaboratively with residential staff, healthcare providers, and support teams.
This is an excellent opportunity for a nurse seeking a flexible schedule, supplemental income, and meaningful work in a person-centered environment. The position typically requires fewer than 20 hours per month, with primary availability needed during the first week of each month for scheduled wellness checks.

Conduct monthly wellness checks during the first week of each month, including obtaining vital signs and documenting findings.

Monitor resident health status and communicate changes, concerns, and follow-up needs to appropriate team members.

Provide nursing support related to residents’ health needs and individualized care.

Provide staff education and support related to medication administration, including topical medication training and reinforcement of proper medication administration procedures.

Assist Direct Support Professionals (DSPs) with medication administration questions and provide guidance as needed.

Maintain accurate and timely nursing documentation, progress notes, and health records in compliance with HIPAA and ODP standards.

Review physician orders, medical recommendations, and health-related documentation as needed.

Assist with coordination of medical appointments and follow-up needs.

Communicate with healthcare providers, families, and support teams regarding resident health concerns as needed.

Provide nursing guidance during health-related concerns and follow established procedures for reporting incidents.

Support compliance with applicable state regulations and health standards.

Must possess a valid Pennsylvania LPN or RN license.

Experience working with individuals with intellectual and developmental disabilities preferred.

Knowledge of ODP regulations and Chapter 6400 requirements preferred.

Strong documentation, communication, and organizational skills.

Ability to work independently and manage scheduled responsibilities.

Ability to train and educate staff on health-related procedures.

CPR & First Aid Certification (or willingness to obtain).
