Community Care Network is looking for an Advanced Practice Registered Nurse (APRN) to join our team in Rutland, Vermont!
The Advanced Practice Registered Nurse (APRN) collaborates with an interdisciplinary team to deliver integrated care for adults with serious mental illness. The APRN builds therapeutic relationships to support treatment goals while balancing independent clinical judgment with provider consultation. Responsibilities include psychiatric care and medication-assisted treatment (MAT) for clients with co-occurring mental health and substance use disorders.
Advanced Practice Registered Nurse (APRN) Responsibilities:
- Conduct initial and ongoing clinical assessments and develop individualized treatment plans.
- Provide psychiatric evaluation, medication management, and MAT, with ongoing monitoring in coordination with primary care providers.
- Participate in on-call coverage.
- Lead and collaborate within an interdisciplinary team to ensure coordinated, responsive care.
- Communicate changes in treatment, medical, and service needs to relevant staff.
- Maintain accurate, timely documentation in the electronic medical record.
- Ensure compliance with all policies, procedures, and psychiatric service protocols.
- Support agency operations through clear communication, external partnerships, and a culture of quality improvement.
- Attend required meetings and trainings; adhere to all agency policies, code of conduct, ethics, and health and safety requirements.
Advanced Practice Registered Nurse (APRN) Qualifications:
- Master’s Degree in Nursing along with the appropriate Nurse Practitioner certification required.
- Psychiatric certification (or actively pursuing).
- Current, unrestricted Vermont APRN license (or ability to obtain upon hire).
- Experience providing psychiatric care and MAT for adults with serious mental illness and co-occurring substance use disorders.
- Experience working with complex populations, including developmental disabilities and medical comorbidities.
- Strong knowledge of psychiatric medications and proficiency with electronic medical records.
- Demonstrated compassion, sound judgment, and strong clinical reasoning skills.
- Must have a valid driver’s license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
- Successful completion of a criminal history background check after hire.
