We are seeking a compassionate and dedicated Hospice Registered Nurse (RN) Case Manager with at least one year of experience to join our team. Responsibilities
- Conduct comprehensive patient assessments to determine care needs.
- Develop and implement individualized care plans that align with patient goals, medical requirements, and hospice guidelines.
- Collaborate with interdisciplinary teams—including physicians, social workers, and family members—to ensure seamless delivery of hospice services.
- Provide patient education on disease processes, medication management, and end-of-life preferences while supporting emotional and behavioral health needs.
Experience
- Minimum of one year of nursing experience as a RN Case Manager in Hospice.

Pay: $82,000.00 - $87,000.00 per year
