Title: INFECTION PREVENTION/INFECTION CONTROL (LVN)
Status: Full-Time, Exempt
Reports To: Director of Nursing
Pay Range: $77k-$87k annually

You will lead with both heart and discipline. That means becoming the facility's central expert for all infection control matters, staying ahead of CDC, CMS, and OSHA regulations, and creating a culture where every staff member prioritizes infection prevention. WHAT YOU BRING TO THE FACILITY
- THE FOUNDATION
- Current, unencumbered Licensed Vocational Nurse (LVN) license issued by the State.
- License in good standing with no active probation or substantiated violations.
- Completion of, or willingness to complete, a recognized Infection Preventionist training course (e.g., from APIC or CDC) within 6 months of hire.
- THE JOURNEY
- Minimum 2 years of clinical experience in a skilled nursing facility.
- Prior experience in an infection prevention role or demonstrated knowledge of infection control practices is highly preferred.
- Proven track record of maintaining regulatory compliance in infection control.
- Experience with outbreak management, contact tracing, and public health reporting.
- THE CRAFT
- Deep fluency in CDC, CMS, and OSHA regulations related to infection prevention and control.
- Ability to develop, implement, and annually evaluate the facility's Infection Prevention and Control Program.
- Strong surveillance and data tracking skills – you can identify trends and implement interventions.
- Excellent communication and teaching skills – you can educate and influence staff at all levels.
- Meticulous attention to detail and strong analytical skills to interpret infection data.
- THE CHARACTER

WHAT YOU WILL BE DOING
- Develop, implement, and annually evaluate the facility's Infection Prevention and Control Program.
- Conduct ongoing surveillance of healthcare‑associated infections (HAIs) using established criteria.
- Track, analyze, and report infection data to the QAPI committee.
- Lead the facility's response to any infection outbreaks, including contact tracing, implementing containment measures, and communicating with the Department of Public Health.
- Ensure infection control policies and procedures are current, evidence‑based, and in compliance with CDC, CMS, and OSHA regulations.
- Serve as the primary educator for all staff on infection control practices, including hand hygiene, standard and transmission‑based precautions, and emerging infectious disease threats.
- Maintain a state of continuous survey readiness for all infection control‑related standards.
- Act as the facility's subject matter expert during internal and external surveys.
