Clinical Liaison – Home Health & Hospice

Are you a clinically trained professional who thrives on building relationships and driving meaningful impact? We’re seeking a Clinical Liaison – Home Health & Hospice to serve as a trusted resource to hospitals, physicians, and care teams while helping patients transition smoothly into home health or hospice services.
In this role, you’ll combine clinical expertise with relationship-based outreach to grow referrals, support compliance, and strengthen partnerships across the healthcare continuum.
What You’ll Do

Build and maintain strong referral relationships with hospitals, SNFs, physicians, and healthcare partners

Assess patient eligibility for home health and hospice services in accordance with clinical and regulatory guidelines

Collaborate closely with discharge planners, case managers, and physicians to ensure seamless transitions of care

Educate patients and families on services, care plans, and expectations

Drive referral growth by consistently achieving established goals

Identify and develop new growth opportunities with hospitals and health systems

Coordinate and support C-suite and leadership-level meetings to strengthen hospital partnerships

Partner with leadership to establish preferred partnerships, ACO relationships, narrow networks, and electronic referral integrations

Ensure referrals meet all regulatory, payer, and compliance requirements

Work with clinical teams to support timely start of care

Provide education and in-services to referral sources on eligibility, regulations, and best practices

Represent the organization at community events, professional meetings, and networking opportunities
What We’re Looking For
-
RN, LPN, PT, OT, or other qualified clinical background preferred
-
Minimum of 2 years of experience in healthcare (home health, hospice, or discharge planning preferred)

Strong understanding of home health and hospice regulations, Medicare/Medicaid guidelines, and reimbursement
-
Proven ability to build trust-based relationships with providers and healthcare partners

Comfortable working independently while collaborating with interdisciplinary teams

Strong communication, presentation, and organizational skills

Proficiency with EHR systems and Microsoft Office

Ability to travel routinely within assigned territory

Valid driver’s license with reliable, insured transportation
