Responsibilities
- Provide comprehensive patient care, including assessments, vital signs, medication administration, and wound care in accordance with physician orders and facility protocols.
- Administer IV infusions, injections, and perform catheterizations while ensuring sterile techniques and safety standards are maintained.
- Utilize EMR (Electronic Medical Records) systems such as Epic or Cerner to document patient information accurately and efficiently.
- Conduct patient evaluations for acute pain management, dementia care, Alzheimer's care, and memory support, tailoring interventions to individual needs.
- Collaborate with multidisciplinary teams to develop and implement personalized care plans for patients with complex conditions such as infectious diseases or post-acute needs.
- Assist in triage assessments, determining urgency levels for patients in urgent care or emergency settings, including Level I Trauma Centers or ICU environments.
- Support patients across various settings including skilled nursing facilities, assisted living communities, home health visits, hospitals, and outpatient clinics.
- Ensure compliance with HIPAA regulations and maintain clinical confidentiality policies at all times.
- Communicate effectively with clients and caregivers, providing education on medication administration, disease management, and health coaching.
- Participate in discharge planning and case management activities to promote smooth transitions of care.
Experience
- Valid LPN license with current state licensure required; compact license preferred for travel flexibility.
- Proven experience working in diverse healthcare environments such as hospitals (including ICU, ER, OR), long-term care facilities (nursing homes, skilled nursing facilities), assisted living communities, or home care settings.
- Critical care experience highly valued, including ICU or Level I Trauma Center exposure.
- Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks is essential for documentation accuracy.
- Experience working with individuals with disabilities or special needs such as dementia or Alzheimer's disease is preferred.
- Knowledge of ICD coding (ICD-9/10), CPT coding, and medical terminology enhances your ability to deliver precise patient documentation.
- Strong assessment skills in geriatrics, pediatrics (including neonatal ventilator management), or behavioral health are advantageous.
- Ability to adapt quickly to new environments while maintaining high standards of patient safety and quality care.
