- Case Management experience a plus.
- Must live within 20 miles of downtown Orlando.
- Must have knowledge of community resources.
- Marketing experience is a plus.
- Flexibility is a must.
- On call is required one weekend a month.
- Ability to travel locally is required. Must have reliable transportation.
- This is a part time position up to 20 hrs a week.
Responsibilities:
- Conduct professional client Assessments and develop a care plan based on goals and needs of the client
- Provide resources and support for clients and their families by identifying needs
- Maintain client case load by providing ongoing Care Management by coordinating care
- Maintain billable hours and report in a timely and accurate manner to meet deadlines
- Work well independently and as a team with other like professionals
- Demonstrate excellent communication skills
- Able to adapt to different environments, diverse individuals, and family systems
- Present a professional appearance that reflects the company image
- Be organized and proficient in Word, Outlook
- Committed to accurate, timely updating of client records
- Demonstrate dynamic community leadership skills
Qualifications:
Must have a Master's Degree in the field of Social Services or Human Services
Minimum of two (2) years of relevant experience in Health Care, Mental Health, or the Social Service setting to be considered.
